Dr. Brian Eiss, Co-Chief Resident, Department of Medicine, and Dr. Judy Tung, Director of WCIMA and Acting Chief, Division of General Internal Medicine, presented a workshop (attended by over 50 people) at the Association of Program Directors of Internal Medicine (APDIM) Spring Meeting in Orlando. The session was held in conjunction with clinician educators from University of Chicago and the novel model presented addressed the activity of handing patients off (from graduating senior residents to incoming interns) in the resident outpatient practice.

Dr. Eiss explains: "Many residency programs across the nation do not have models for ensuring comprehensive, safe handoff of residents' outpatient panels, which would allow for opportunities to share in the work done at programs such as ours at NYP/WC. Typically, there have been no formal processes in place. At WCIMA (NYP/WC), senior medicine residents are asked to create a living sign-out document that lists all patients under their care and identifies concrete tasks that require follow-up to occur within the immediate transition period for incoming interns. High-risk patients are identified ahead of time and are usually reassigned to junior residents. This new model seeks to improve patient safety and education."
